Where everyone comes together to support one person’s needs.
Within the various sectors of Social Services exist mechanisms for System Coordination. While each sector may refer to the process by different names, overall the goal is the same which is to collaborate with Service Providers in order to more adequately support persons with complex needs. The Central West & Mississauga/Halton Local Health Integration Network (LHIN), Ministry of Children Community and Social Services (MCCSS), all fund such coordinating efforts with staffing and financial resources. Some of these funded efforts have existed for many years, while others are relatively new.
In the Peel Region, although not mandated, the staff involved in “System Coordination-type work” voluntarily agreed to collaborate as a network in order to better centralize System Coordination access and maximize resources, knowledge, and problem-solving.
The network adopted the name SystemWise. We represent coordination efforts for persons with complex needs from the following Child, Youth, and Adult Social Service sectors: Acquired Brain Injury, Addictions & Mental Health, Criminal Justice, Developmental Disabilities, and Dual Diagnosis,
Who is eligible?
*Transitional Aged Youth 16-24 (TAY)
The goal of CMHA Peel Dufferin’s piece of System Coordination is to facilitate access and/or develop community plans for TAY with complex mental health and/or addictions challenges to care that is coordinated uninterrupted, developmentally appropriate, comprehensive and flexibly adapted to the extraordinary needs of the youth.
